Mechanism of actionBinds the 30S ribosomal subunit, inhibiting bacterial protein synthesis; strong Gram-negative (incl. Pseudomonas) coverage, variable staph, poor streptococcal/anaerobic.
Indication / reason for useBacterial conjunctivitis and blepharitis; component of fortified regimens for keratitis; surgical prophylaxis.
Usual adult dosingMild-moderate: 1-2 drops q4h. Severe: 2 drops q30-60 min, then reduce. Ointment BID-TID.
PreservativeBAK 0.01%
Contact lensesNo lens wear during infection.
Common / important adverse effectsPunctate epithelial keratopathy and lid/conjunctival hypersensitivity with prolonged use (aminoglycoside toxicity), redness.
Contraindications & cautionsAminoglycoside hypersensitivity.
Pregnancy / lactationLimited data; topical exposure low.
Practice notesSurface toxicity is common after ~1 week, don't continue courses indefinitely.
