Mechanism of actionCompetitive inhibitor of PABA in folate synthesis; bacteriostatic, broad but with substantial resistance today.
Indication / reason for useBacterial conjunctivitis and blepharitis; adjunct in trachoma.
Usual adult dosing1-2 drops q2-3h initially, tapering; ointment QID + bedtime.
PreservativeBAK / thimerosal depending on brand
Contact lensesNo lens wear during infection.
Common / important adverse effectsStinging (acidic), hypersensitivity including Stevens-Johnson syndrome (rare but serious), local irritation.
Contraindications & cautionsSulfonamide hypersensitivity.
Pregnancy / lactationAvoid near term (kernicterus concern with sulfonamides).
Practice notesLargely superseded by fluoroquinolones and polymyxin combinations.
