Mechanism of actionCalcineurin inhibitor; blocks T-cell activation and IL-2 transcription, reducing ocular surface inflammation and increasing goblet-cell density and tear production.
Indication / reason for useChronic DED with an inflammatory component; increases tear production where suppressed by keratoconjunctivitis sicca. Verkazia/Vevye for vernal keratoconjunctivitis.
Usual adult dosing1 drop in each eye twice daily, ~12 h apart. Onset of benefit typically 1-3 months.
PreservativePreservative-free single-use vials (Restasis MultiDose bottle uses a PF valve) , a preservative-free option exists
Contact lensesRemove lenses before instillation; may reinsert 15 minutes later.
Common / important adverse effectsBurning/stinging on instillation (common, transient), conjunctival hyperemia, foreign-body sensation.
Contraindications & cautionsActive ocular infection; caution with herpetic keratitis history.
Pregnancy / lactationLimited data; systemic levels typically undetectable. Use if clearly needed.
Practice notesSet expectations about delayed onset and initial stinging to aid adherence; a short topical steroid pulse can bridge the first weeks.
